At Mountainside, our continuing care case managers help you decide what is the best next step in your recovery. Through one-on-one sessions with your case manager, you will create a detailed aftercare plan that enables you to build on the progress you have made in treatment. Your case manager then coordinates with your next treatment and care providers to make your transition as stress-free as possible.
Our goal is not only to help you achieve sobriety but to help you successfully adjust to your new sober life outside of treatment. Your case manager will work closely with you, your clinician, and your loved ones to ensure that you have the tools and support to move forward in your recovery.

Understanding what you need to stay on the right track is essential to your recovery. Through our educational offerings you identify your strengths and weaknesses, determine the best way to achieve your goals, and gain the knowledge and tools to overcome the challenges of early recovery.
Based on your strengths, progress, and goals, your case manager shares options for the next addiction treatment steps after you complete your current phase of drug or alcohol rehabilitation.
To better prepare you for what life is like after treatment, you participate in a discussion group, addressing daily challenges you may face, where to find helpful resources, and more.

To ensure that you continue to receive quality treatment, your case manager vets and connects you with any service providers you may need once you leave Mountainside ensuring a smooth transition.

Before you leave Mountainside, you will be equipped with the necessary tools and resources to succeed, including information to local 12 Step meetings, referrals to wellness therapies, and access to our Alumni Services department, ensuring you receive continued support long after you have completed treatment.
Continuing Care team members will check up on your health and well-being, connect you with the recovery community, inform you of alumni events, and encourage you to stay motivated in your recovery.
